Cougar athletes are a unique group of individuals. Not only are they positive role models and an asset to the BYU community, they are also Conference Champions, All-Americans and National Champions. As a result, student-athletes and coaches are often asked to appear at or be involved in charity or educational functions.

Examples where the appearance, picture or name of a student-athlete or coach may be requested includes, but is not limited to:

• Providing education to a class about the importance of staying in school or staying away from drugs through The Buff Don't Puff anti-smoking campaign
• Visiting retirement or rehabilitation centers
• Attending sporting events sponsored by the Boys and Girls Club
• Speaking at Church firesides or Church other functions
• The use of a student-athlete's name or picture to promote any item or event
• The use of a student-athlete's name or picture in connection with any commercial item or agency
• Any other activity or situation where the name, picture or appearance of a student-athlete will be used

Please Note:
In order to assure compliance with NCAA regulations, the BYU Athletic Department requires all individuals or organizations to submit requests for such activities by using this form.  This online form helps the BYU Compliance Office review the specific activity for permissibility within the NCAA rules. 

Submitting your request is simple and will only take a few minutes.  Upon approval from the Compliance Office, the request will be forwarded to the appropriate person(s) in charge of coordinating such an activity.  Approval from the Compliance Office however, does not grant the request.  Final approval of the request rests with the respective offices involved.  Please allow 2-3 weeks for requests to be processed.
