Registration Deadline: Friday, February 18, 2022 (no exceptions)
Requirements:
- Must be currently attending BYU and enrolled in at least 12 credits
- Must provide a copy of a completed Covid Vaccine card OR provide a Negative Covid Test Result that is within 3 days of the tryout. Tests can be done through the university
- Need to obtain a signature on your tryout packet from the Football Compliance Officer, Kurt Hunsaker, Office: (801) 422-8986 (located on 2nd floor of SAB in Football Offices). You DO NOT need the coach's signature.
- Turn in completed Tryout Packet with required Compliance signature to Candace Gunn in the Football Training Room, SAB 123
- Select to print Required BYU Football Walk-on Tryout Registration Packet
- Select to print Required BYU Football Walk-on Tryout Registration Packet
- Complete and submit BYU Football Questionnaire
- Select to fill out BYU Football Player Questionnaire
- Select to fill out BYU Football Player Questionnaire
- Provide proof of current medical insurance (pics or copy of cards)
- Please note the Student Health Plan (SHP) offered by the university does not cover athletic related injuries
- Insurance should be under your parents, yourself, or your spouse through another plan
- If you need assistance with insurance, see Candace Gunn (contact info listed below)
- Provide a copy of a recent athlete physical exam (completed within the last 3 months)

- Select to print Sample Physical Exam Form (that can be used if your doctor doesn't have one)
- Provide a Sickle Cell Blood Test Result
- This is a one-time blood test called a Hemoglobin S Evaluation that can be done at your Athlete physical appointment or can be from a previous test you already had done
- Please note these results typically take 2-3 days to come back from the lab
- Must adhere to the BYU grooming standards & Honor Code
All of the above items MUST be completed and submitted by February 18 to be able to participate in the tryout. No exceptions! The tryout will include combine-style drills and position workouts. BYU's strength and condition coaches will conduct a warmup session with all participants at the beginning of the tryout. Registered participants should bring their own workout gear and cleats.
